Description

Job Summary

Assumes responsibility in association with physicians and leaders for a caseload of oncology patients with consultation, education, collaboration and medical supervision. Assesses, plans, implements, evaluates, anticipates, and supervises survivorship care according to the organizations policies and procedures. Participates in the development of guidelines for oncological survivorship to meet the needs of the patient. Serves as a resource for the Cancer Committee through consultation, education and research at various levels to continuously improve the survivorship process. Participates in the improvement of operational functions of the Department of Oncology. Establishes communication and respectful relationships with all disciplines.

Job Specific Duties

  • Delivers patient-centered care in accordance with standards of care defined by Nicklaus Children’s Hospital and clinical practice guidelines from COG.
  • Provides treatment to patients in all regards of survivorship, including: survivorship treatment, long-term and late effects of cancer and its treatment, long-term and late effects of bone marrow transplant and its treatment, surveillance testing and exams, signs of recurrence, nutrition management, healthy lifestyle behaviors as well as support services referrals.
  • Facilitates patient-family encounters and assists the healthcare team in development and implementation of a comprehensive survivorship care plan and provides the patient with summary in regards to the COC policy. 
  • Communicates with primary physician’s office and other providers for the patient’s and physician’s needs.  Serves as a liaison to community organizations for ongoing information and support services.
  • Ensures communication between team members; proactively schedules appointments & follow-ups; serves as a front-line resource to obtain records, test results, & other clinical information prior to clinic visit.
  • Prepares and maintains accurate patient records, charts, and documents to support the medical practice.  
  • Facilitates the seamless transition from pediatric to adult survivorship care.  
  • Collaborates in the screening and referral of patients for fertility preservation.  
  • Provides system-wide training, clinical practice, as well as, staff development pertaining to survivorship.
  • Systematically evaluates survivorship care standards; makes process improvement recommendations. Utilizes data to develop process improvement plans to improve clinical outcomes.  
  • Presents results, trends, or conclusions from analysis of performance improvement initiatives. 
  • Assures compliance with DNV, Commission on Cancer, FACT and other regulatory standards.  
  • Participates in the improvement of operational functions of the department as well as providing the Cancer Committee with an annual evaluation of survivorship processes and outcomes. 
  • Works closely with the Physician Leaders, Service Line Administrator, Nursing Manager, & other leaders.
  • Participates in developing, educating, and integrating system-wide survivorship clinical policies and documentation tools.
  • On an aggregate level, tracks performance of the program in line with (NCH), division and facility goals (growth, quality, practice guidelines, etc.) and identifies opportunities to streamline care practice through development of evidence-based outcomes; responsible for USNWR queries. 
  • Attends survivorship program seminars/conferences to maintain knowledge base regarding palliative care.  
  • Maintains communications with administrators, physicians, clinicians, patients, and families both verbally an in writing.   
  • Acquires and maintains current knowledge in nursing practice and education.

Qualifications

Minimum Job Requirements

  • Bachelor's Degree Nursing
  • RN Licensure within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) – maintain active and in good standing throughout employment
  • American Heart Association BLS– maintain active and in good standing throughout employment
  • APHON Chemotherapy Provider
  • CPHON
  • 3-5 years of experience in Oncology
  • 1 year of experience as a nurse navigator and/or patient educator in Oncology
  • 1-year of hands-on experience in program development

Knowledge, Skills, and Abilities

  • Master's Degree in Nursing preferred
  • Bilingual in English/Spanish preferred
  • Knowledgeable regarding developmental issues and a unique awareness of the concerns pertaining to adolescents and young adults preferred
  • Ability to utilize critical thinking and problem solving skills to identify, interpret, and access appropriate resources in patient specific scenario(s)
  • Strong interpersonal, communication, collaboration and conflict resolution skills
  • Good analytical, deduction, and anticipation skills
  • Working knowledge of Microsoft office products
