Patient Flow Coordinator- Emergency Dept (Part Time, Evening shift)

Job Summary

Monitors patient flow that includes admission, transfers and patients discharged from the department. Collaborates and communicates with the Clinical Coordinators, Nurses and Medical Team to facilitate timely movement of patients through the department. Assists with the flow of patients from triage to the different areas of the ED based on mode of arrival, patient condition and chief complaint. Supports the ED initiatives and goal metrics including turn-around-times for arrival to triage, admit orders to disposition, and overall Length of Stay.  

**Anticipated schedule for this position is: 12:30pm to 1:00 am



Job Specific Duties

  • Coordinates and continuously monitors patient flow, supporting patient throughout utilizing appropriate resources from admission, transfer or discharge, and facilitates the processes in a timely manner.
  • Identifies barriers to the admission/transfer process for patients. Suggests solutions to identified barriers and in conjunction with ED management, implements these solutions.
  • Develops and promotes positive effective customer service relations with patients, patient family members, visitors, and staff. Ensures maximum staff productivity.
  • Works closely with ED Physicians to identify patients eligible and appropriate for results pending to facilitate the flow of patients throughout the ED.
  • Manages the flow of incoming EMS & direct transfers.
  • Ensures the department is meeting its metric standards including, but not limited to: door to triage, bed assigned-to-discharge, and door-to-discharge.
  • Maintains an awareness of and assists the Department in meeting its throughput metrics (e.g.: LWBS, LOS).
  • Participates in ED councils, and assists and supports performance improvement or lean initiative related to Emergency Department flow.

Qualifications

Minimum Job Requirements

  • A minimum of one year healthcare or administrative experience.

Knowledge, Skills, and Abilities

  • Associates degree preferred.
  • Strong customer service, problem solving, critical thinking & conflict resolution skills.
  • Strong communication and influencing skills.
  • Ability to manage multiple priorities and strong time management skills.
  • Effective verbal and written communication skills.
  • Able to learn appropriate software application(s).
  • Able to work collaboratively and follow the direction of the Registered Nurse, Clinical Coordinator and/or Nursing Leader.
  • Able to work collaboratively in a team-oriented environment.
  • Dedicated to providing excellent customer service and meeting customer expectations.
